He is less worried for me. I am officially on Active Surveillance (AS).

He also suggests it can be somewhat common to see one drop, but in my case, I have never had my PSA drop since my tracking began in November 2017. Maybe stopping the testosterone supplement is the cause, although my testosterone is still in the 300s. This is puzzling since my urologist said while taking testosterone supplements, my body stops making it, and stopping it would normally mean less testosterone until my body revs back up again. Can I make the drop happen in my PSA a second time? That would really mean something.

My urologist goes on to say I should retest my PSA again in four months, and then four more months to my next MRI which will tell us a lot at that point. The MRI will measure my tumor diameter, which was 1.4 cm last November. Tumors are spherical, shaped like a marble, and the volume is in direct proportion to the PSA number, my urologist tells me. My PSA has dropped in half. Will my tumor now measure at half the volume? The MRI hopefully will confirm that. That would be scored as “in remission” meaning not progressing. As a former math teacher, the volume of a sphere as a function of the radius was never so useful. Previously to this, I was taking one a day. I keep hunting down more and more articles that confirm what I already think is a good course. More and more great news on the pomegranate seed oil (PSO), so I’ve been increasing that to three half-droppers full twice a day, always taken with the other supplements luteolin and ellagic acid. I learned that prostate cancer research takes place on several types of prostate cancers called prostate cancer lines. Each line is a cancer from a particular individual in the past. The line is used and reused for research consistency, but is not the exact cancer found in anyone else’s body, yet similar. For example, the LNCaP line came from a 50-year-old caucasian male in 1977. The whys and wherefores of using particular cell lines to study can be found here as well as the links below.

LNCaP: This cell line can be used to model different aspects of prostate cancer progression, such as hormone-dependent and castration-resistant cancer.

PC-3: This cell line is useful for studying biochemical changes in advanced prostate cancer cells and how they respond to chemotherapy.

DU-145: This cell line is also commonly used in prostate cancer research.

These models are designed by transplanting cells from human prostate cancer cell lines into immunocompromised mice. Prostate cancer is discussed in section 3.2. In 3.2 it states, “A concentration of 50 μM ellagic acid mitigated PC-3 proliferation“. Mitigating proliferation means “slows down the rapid growth”.

Ursolic acid also seems to work wonders, but researchers fret over its lack of water solubility and hence its bioavailability. And it leaves the body within 4.5 hours. So while it works great at killing prostate cancer, it’s hard to get into your blood, and when you get that, it leaves so quickly.

I wonder to myself, which of the five cancer cell types do I have? So I ask my urologist through the MyChart portal. My urologist responds that he does not know this information, and that the acronyms I am using are research literature focused. Maybe if I reach out to the lab which examined my biopsy samples, they may know something. I later learn as explained earlier that cell lines are cancers extracted from people in the past, and used and reused in university research for experimental consistency.

I read more about it, I learned the five (and more!) cell lines are from already metastasized PCs, and there are still questions about how effective the results of the research can be applied to localized PC. I am getting the feeling that my localized PC is of a different form than any of the research cancers. But what choice do I have? Left untreated by my experiment of one (myself!), my PSA rises, and my cancer has been progressing. Now my PSA has started to drop. I fleetingly think about subdividing my treatment to potentially discover which part of the treatment may be more effective than other parts, but that quickly leaves my mind. All I want to do is keep driving the PSA down for now. I will have time for these other sorts of experiments on myself later, after my PSA is under 2.0 ng/mL and the presence of PC is physically determined to be dropping, hopefully by MRI alone. My biopsy aftershock is something I hope to never have to repeat.

Closing Thoughts

In summary:

Both my Urologist and PCP say congratulations on the PSA drop, but do it again to be meaningful.

Changes in PSA in known PC patients is directly proportional to tumor size.

I have not been experiencing any side effects other than some extra gas. So I double my supplements to once in the morning, and once at night.

PSO emerges as a powerful PC fighter. I always take it with ellagic acid and luteolin.

I would like to know which of my multiple strategies is making my PSA drop. But I decide it is too early to not just keep doing the same thing.

Stay the course: I can avoid surgery and radiation if the current trend continues.

Month 4 begins with the teleconference with my urologist, and he says congratulations on the PSA number drop.
